Responsibilities

This position will work with adult patients in both our regular and co-occuring disorders programs.

Hours are Monday - Friday, 8a-430p

Responsibilities and Accountabilities :

• Accountable to have knowledge and understanding of Social Work practices and principles
• Responsible for effective case psychosocial assessment and suicide risk assessment, discharge planning, and leading psychoeducational and / or therapeutic groups
• Responsible to develop treatment plans, complete discharge plans and conduct discharge meetings
• Responsible for communicating with patients and families to keep them involved in treatment plan decisions
• Coordinates care with outpatient providers upon admission and discharge
• Responsible for facilitating weekly treatment team meetings and creating master treatment plans / updating treatment plans as agreed upon by the treatment team
• Contributes clinical perspective to the interdisciplinary team based on unique training in the area of family systems and / or social work theory.
• Communicate clearly and effectively with clinical team members and Directors
• Is oriented and educated in gender, cultural differences, and age appropriate care for the assigned department

Qualifications

Compensation for this role ranges from $29- $36 per hour depending on licensure and experience.
