ABOUT CPLC LUCES
CPLC LUCES (Latinos Unidos Contra El Sida / Latinos United Against Aids) provides HIV prevention education and supportive services to people living with or at risk of HIV / AIDS to improve health outcomes, reduce health disparities, and address barriers to treatment.
COMMUNITY-BASE ROLE
The case manager engages with clients in different settings, including, but not limited to, the office, homes, offsite community and / or clinical locations. This role also attends primary medical care appointments with clients as needed. The case manager travels throughout Maricopa and Pinal County, utilizing an insured personal vehicle.
ABOUT THE ROLE
Under general supervision, provides direct covered services to individual clients, coordinates with community support and stakeholders. Responsible for managing appointment schedule and assigned caseload, referrals to medical services, conducting needs assessments, implementing treatment plans, facilitating referrals, managing clinical care team, and participating in ongoing learning experiences.
