POSITION SUMMARY :

The Licensed Counselor works as an embedded member of the primary care team, providing brief behavioral health interventions. This fast-paced clinic role focuses on same-day consultations, rapid assessments, and seamless collaboration with medical providers to address patients' mental health and substance use concerns. The position emphasizes short-term, evidence-based interventions and care coordination rather than the traditional therapy session, while incorporating culturally responsive care that honors Indigenous values and practices.

DUTIES A N D RESPONSIBILITIES

• Provide integrated behavioral health services to individuals, couples, families, and within groups using a variety of evidence-based practices to manage symptoms, enhance coping skills, and promote behavior change.
• Provide comprehensive assessments to identify mental health and / or substance use disorders impacting patients.
• Provide accurate diagnoses based on current criteria from the Diagnostic and Statistical Manual of Mental Health Disorders (DSM).
• Provide target, short-term interventions that are goal-oriented, time-limited, and designed to address immediate concerns or emerging mental health needs
• Collaborate with the patient and care team to ensure continuity of care and determine if longer-term therapy or additional services are needed.
• Provide longer-term outpatient therapy to select patients as appropriate.
• Provide crisis intervention intended to reduce potential irreversible adverse effects to an individual affected by a crisis and guide staff on appropriate responses.
• Provide care coordination, education, referrals, advocacy, and other related therapeutic services.
• Complete and maintain accurate, timely documentation of each encounter of therapeutic services in the electronic medical record (EMR).
• Maintain current knowledge of behavioral health and integrated healthcare issues, impact on practice, and best practices.
• Maintain compliance with local, state, and federal standards / rules as well as organizational mission, values, policies, and procedures.
• Maintain confidentiality in matters related to patients, and families.
• Support overall clinic workflows and participate in quality improvement initiatives as needed.
• Provide culturally competent, trauma-informed care.
• Other duties as assigned.

PHYSICAL DEMANDS

Essential job duties may require occasional lifting, pushing, and pulling medical or educational supplies and equipment. The ability to occasionally stoop, kneel, crouch, and utilize reaching, handling, fingering, and feeling movements is required. The ability to hear normal conversational discussion and near and far visual clarity, or equivalent adaptation, is required. The employee may risk exposure to potentially dangerous situations, including exposure to communicable diseases.
