VHS is looking for a qualified Nurse Practitioner - Family Medicine.

• Description : Chart closure within 72 hours Schedule = 18 PPD, minimum is 18 PPDMonday-Friday 8-5, open to 10 hour shiftsMix of peds / adultsMust have at least 3 years experience in similar setting (panel management) Needs experience with Chronic Pain ManagementBE will be considered if truly BE (5 years or less since residency)Active DEA needed before startCredentialing takes 45-60 daysThe following immunizations are required for credentialing (before start date) : Flu Vaccine, TB Status / X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years),
• 1099 Pay Rate : Competitive Market Rates (W-2 Pay Packages available upon request)
