  • Work Dates Needed : December 8 (or with -day credentials) - 3 months
  • Preferred Schedule : M-F 8a-5p
  • Worksite Setting : Clinic
  • Scope of Work : Mix of peds / adults. 18 PPD, minimum is 18 PPD. Includes some Chronic Pain Management
  • Licenses, Certifications, Requirements : Active CA license, BLS, PALS, Board certified

    • The following immunizations are required for credentialing (before start date) : Flu Vaccine, TB Status / X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years). Covid-19 Vaccine no longer required
