MORE ABOUT THE PT PHYSICAL THERAPIST POSITION :

Responsibilities for this Physical Therapist opportunity include, but are not limited to :

• Provides Physical Therapy services to patients under the appropriate guidelines established by each individual client facility.
• As requested by the client facility, conducts screening of patients in accordance with the policies of the client facility to determine need for intervention / treatment.
• Evaluates patients to obtain data necessary for treatment planning and implementation. Interprets and communicates evaluation findings to patients, family, and other team members.
• Establishes individual patient schedules in conjunction with other team members.
• Develops treatment plans by identifying a problem list, including short and long-term goals and methods to achieve identified goals.
• Coordinates treatment plan with patient, family, and other team members. Implements and conducts treatment as outlined in treatment plan.
• Monitors patient's response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders.
• Develops appropriate home or community programming to maintain and enhance the performance of the patient in his or her own environment
• Documents results of patient's assessment, treat ment, follow-up, and termination of services.
• Adapts treatment to meet the individual needs of the patient.
• Provides / recommends / constructs adaptive devices, orthotics or prosthetics.
• Trains patient, family and appropriate staff in the use or application of the above. Participates in discharge planning when applicable.
• Supervises and directs Physical Therapy Assistants in the implementation ofthe treatment plan, when applicable.
• Supervises students and volunteers, when applicable.
• Supervises Physical Therapy Aides and Rehabilitation Aides, when applicable.
• Reviews the quality and appropriateness of the total services delivered and of physical therapy programs for effectiveness and efficiency, using predetermined criteria.

JOB REQUIREMENTS FOR THE PT PHYSICAL THERAPIST :

• Masters of Science or Doctorate in Physical Therapy from an accredited program
• S uccessful completion of National Certification Examination for Registered Physical Therapist.
• 1 year of verifiable, supervised professional experience.
• Current state licensure
