No Patients Per Shift : 16 Bed In Department : n / a Rounding : N / A Does your Facility require physicians to be board certified? Yes Fellowship : N / A Minimum Board Certification : Eligible The Following Certification Are Required : ACLS : No ATLS : No BLS : Yes PALS : Yes Only allow bids from providers with active licenses in CA? Yes Allow Bids from Providers with IMLC? No Will you reimburse Vendor for costs associated with licensing Provider? No Estimated Credentialing Timeframe (Days) : 60+ Candidate CVs need to have the month & date on their work experience w / all gaps in employment accounted for. Includes some Chronic Pain Management Chart closure within 72 hours Schedule = 18 PPD, minimum see 18 PPD Monday-Friday 8-5 A mix of peds / adults (Will consider IM only) A decision is typically made within 7-14 days We will also accept DO BE will be considered if truly BE (5 years or less since residency) Active DEA needed before the start Credentialing takes 30-90 days (six weeks is common if the provider submit app. / supporting documents timely) The following immunizations are required for credentialing (before start date) : Flu Vaccine, TB Status / X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years).
