Type : Full Time

Salary : Starting at $55 / Hour, depending on experience

Job Description :

• Assess patient and family to determine home care needs
• Provide physical assessment and history of illness(es)
• Initiate and revise plan of care as needed
• Develop care plan with patient and family involvement
• Identify discharge planning needs and implement prior to discharge
• Counsel patient and family in meeting nursing and related needs
