About the Role :
The Neighborhood Outreach Worker (NOW) is for the Maternal and Infant Outreach Program. Under supervision, the Neighborhood Outreach Worker provides intensive, home-based visits, health education, referrals, advocacy, and support for pregnant and post-partum women and infants. The Neighborhood Outreach Worker also provides comprehensive services to clients during their pregnancy, postpartum, and through the age of 18 months for the baby. NOWs provide emotional and social support; observes case management procedures; assists participants in managing practical problems, finding resources, and empowering participants to make decisions. Performs related work as required.
Minimum Qualifications : .
- High School Diploma or GED and two (2) years of experience providing education, advocacy, direct care services for maternal child health or closely related field, experience working with a diverse population.
- Strong communication and interpersonal skills.
Preferred Qualifications :
Bachelor's degree in social work, public health, or a related field.Bilingual proficiency in English and another language commonly spoken in the community.Experience working with diverse populations and understanding of cultural competency.Responsibilities :
Conduct outreach activities to engage with community members and assess their needs.Provide information and referrals to health and social services available in the area.Organize and facilitate workshops and events to promote health education and awareness.Collaborate with local organizations, healthcare providers, and government agencies to enhance service delivery.Maintain accurate records of outreach activities and client interactions for reporting and evaluation purposes.Skills :
Knowledge of Community Outreach practices.Knowledge of technical platforms (Microsoft Suite, Excel, Teams, etc.)Ability to communicate effectively to maternal participants, providers and staff.Ability to provide person centered services to participants with mental health and substance use diagnoses.Ability to work flexible work hours including pre-schedules nights and weekends.Strong organizational skills.
