Job Description

The Wound Care Nurse Practitioner (NP) reports to the Director of Nursing and supervises nurses and medical assistants.

• Provides specialized wound care in home, nursing home, and assisted living settings for chronic, pressure, surgical, and complex skin wounds.
• Performs assessments, develops care plans, educates patients / caregivers, and collaborates with healthcare teams to promote healing.
• Requires a Master’s or Doctoral degree in Nursing, current NP licensure, and Wound Care Certification within 6 months.
• Minimum 2 years of healthcare experience; active patient contact preferred.
• Must be physically capable, adaptable, and have reliable transportation for travel to patient locations.

Essential functions include wound assessment, treatment, supervision, documentation, and interdisciplinary coordination. The role demands strong clinical skills, interpersonal communication, and flexibility in various environments.
