- Hours : Full-Time, Scheduled 40 hours per week - no hours guaranteed\n
- Schedule : Monday-Friday, 8 : 00 AM - 5 : 00 PM with occasional weekends and on-call\n
- Productivity : 30-32 per week expected, up to 35 visits\n
- Mileage : .38c visit to visit, 150-250 miles per week expected\n

- Conduct home health visits to patients' homes within the territory and the surrounding area.\n
- Perform comprehensive assessments of patients' health status, including physical, emotional, and environmental factors influencing their well-being.\n
- Develop and execute personalized care plans tailored to meet the unique needs and goals of each patient, in collaboration with the healthcare team and under physician guidance.\n
- Administer skilled nursing care, encompassing wound management, medication administration, IV therapy, and patient education on disease management and preventative care measures.\n
- Maintain accurate and timely documentation of all patient interactions, assessments, interventions, and outcomes utilizing Oasis and Homecare Homebase software platforms.\n
- Foster interdisciplinary collaboration by actively participating in team meetings, sharing insights, and coordinating care transitions to ensure continuity and effectiveness of care delivery.\n
- Establish and maintain open lines of communication with patients, families, and caregivers, providing support, education, and advocacy to enhance patient understanding and engagement in their healthcare journey.\n\nJob Requirements : \n
- Active PennsylvaniaRegistered Nurse (RN) License.\n
- Active American Heart Association (AHA) or American Red Cross Basic Life Support (BLS) certification.\n
- Minimum of 1 year of experience in home health nursing.\n
- Minimum of 1 year of experience with Oasis and Homecare Homebase software.\n
- Oasis certified strongly preferred.\n
- Excellent clinical assessment and critical thinking skills.\n
- Strong communication and interpersonal abilities.\n
