Auburn Crest Hospice is looking for a driven individual who desires to lead, influence, motivate and mentor an interdisciplinary team (IDT) in providing compassionate care with clinical excellence. As the Director of Nursing for Auburn Crest Hospice you will partner with and lead this team of professionals to care for patients facing the end of life.

As the Director of Nursing you will be responsible for the planning, implementation, delivery, and supervision of direct clinical services provided to clients and families. This position reports to the General Manager.

The Director of Nursing for Auburn Crest Hospice is responsible for :

• Providing supervision of the Hospice Clinical Staff including RN Case Managers, LPNs, CNAs, Chaplains, and MSWs ensuring that best practices are delivered throughout the patient and family hospice experience
• Overseeing the IDT process leading all disciplines to ensure quality care coordination
• Promoting a culture of compliance
• Mentoring and training new and existing team members
• Overseeing the QAPI program to ensure accuracy and regulatory fulfillment of timely documentation
• Evaluating all referrals to ensure appropriate admissions for hospice
• Managing client concerns to achieve maximum patient and family satisfaction
• Coordinating and managing contracted agencies and services
• Maintaining a positive work environment for all team members
• Leading the team to excel at the ultimate goal of patient and family satisfaction
