Description

• Conduct thorough evaluations of patients in both inpatient and outpatient environments and create tailored treatment plans
• Implement physical therapy interventions concentrating on enhancing mobility, strength, balance, and overall function
• Work closely with nurses, physicians, case managers, and other rehabilitation professionals to improve outcomes for all
• Educate patients and their families about therapeutic objectives, discharge plans, and home exercise routines
• Accurately document evaluations, treatments, and patient progress in compliance with facility and regulatory requirements
• Engage in care team rounds and interdisciplinary meetings to support coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Specialty certifications (e.g., neurological, geriatric) are beneficial but not mandatory

Skills
