35 hours / week

Case Managers provide differential response for families with children ages 0-5 who are at risk for child abuse and neglect and after care services to children 0 through 17 and their families.  Case management is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual’s and family’s comprehensive needs through communication and resources to promote well-being.

Qualifications :

• Case Manager I - High school diploma or GED required. Bachelor's degree in Counseling, Social Work or related field preferred. Equivalent experience accepted.
• Case Manager II - Bachelor's degree in counseling, social work or related field required. Equivalent experience accepted.
• Knowledge of community resources preferred.
• Bilingual preferred.
• A valid California driver’s license is required of staff who have regular driving duties and the driving record must meet the requirements of their position and be approved by the agency’s insurance company.
