We are seeking a compassionate and experienced temporary Hospice Social Worker to provide short-term support to our hospice team. This role offers the opportunity to make a meaningful impact by supporting patients and families during end of life care. The ideal candidate brings strong clinical social work skills, excellent communication, and a commitment to the hospice philosophy.
Key Responsibilities :
Provide psychosocial assessments for patients and families
Develop and support individualized care plans in collaboration with the interdisciplinary team (IDT)
Offer emotional support, crisis intervention, advance care planning, and resource coordination
Educate patients and families about hospice services, caregiver support, and community resources
Participate in IDT meetings and contribute to patient care planning
Documentation, interventions, and communications within required timeframes and according to regulatory standards
Coordinate with bereavement services as appropriate
Support compliance with Medicare Conditions of Participations (COPs) and agency policies
Maintain strict confidentiality and uphold professional boundaries
Required Qualifications
Bachelors or Masters degree in Social Work (Masters preferred)
Experience in hospice, palliative care, medical social work, or related field
Strong assessment, communication, and clinical documentation skills
Ability to work independently while collaborating with a multidisciplinary team
Valid drivers license with reliable transportation for home visits
