Job Description
The Nurse Practitioner role involves providing equitable, value-based primary care to adult Medicare patients to promote health and prevent hospitalizations.
- Practitioners conduct office visits, wellness exams, and acute care, building trusted patient relationships.
- Collaborate with a multidisciplinary team including physicians, nurses, and support staff in outpatient settings.
- Responsibilities include care coordination, population health initiatives, patient education, and outreach activities.
- Role reports to the Center Medical Director and involves working closely with operational leadership.
- Qualifications include a master's or doctorate in nursing, national certification, active licenses, and work authorization.
- Preferred experience in primary care, geriatrics, or family medicine, with knowledge of coding and population health metrics.
