Mental Health Intake and Outreach Specialist (for ACSW's, APCC. AMFT only)

Position Overview : The Urgent Walk-In Care / Mobile Outreach Coordinator is responsible for triaging urgent walk-in clients, conducting same-day psychiatric and substance use screenings, and referring clients to appropriate services such as BPSR, SUD treatment, medication management (including MAT), psychotherapeutic services, crisis intervention, and withdrawal management and coordinating mobile outreach.

Program Summary : Our client offers outpatient psychiatric and behavioral health services for individuals aged 18+ with severe mental illnesses (SMI) and co-occurring disorders (COD) in San Diego's Central Region. They provide evidence-based services including psycho-social rehabilitation, psychiatric medication management, individual and group counseling, employment support, peer support, urgent walk-in assessments, mobile outreach, case management, MAT, ambulatory withdrawal management, recovery services, and both outpatient and intensive outpatient substance use treatment. Specialized care is available for transitional-aged youth (TAY), perinatal and parenting women, justice-involved individuals, and those with COD. HOC aims to enhance community members' functionality, support their transition to lower care levels, and achieve sustained recovery.

Key Responsibilities :

• Provide acute, same-day multidisciplinary care coordination for clients seeking mental health and / or substance use services.
• Conduct behavioral health assessments, individual counseling, case management, and rehabilitative support.
• Perform crisis intervention as needed.
• Assess client needs for supportive services and make appropriate referrals (e.g., housing, vocational, employment, food).
• Collaborate with service team members on client concerns.
• Maintain detailed documentation of client progress using an electronic health record system.
• Manage client charts and care coordination databases.
• Complete pre-assessments, intake assessments, and other necessary paperwork.
• Enter data and information into appropriate files and systems.
• Perform other duties as assigned.
