In this role, you will :
- Assess patients to identify psychosocial, financial, and environmental needs.
- Conduct initial evaluations and continuous assessments of patient needs.
- Communicate significant findings to relevant personnel to ensure coordinated care.
- Implement safety plans utilizing family and community resources.
- Participate in the development and execution of the Plan of Care to guarantee seamless care and transition.
- Provide skilled interventions aligned with established standards and physician orders.
- Engage patients and families in following care plans and achieving care goals.
- Collaborate with clinical staff to maintain updated patient information and care transitions.

Essential Qualifications :
