Position Summary
The Data Outcome Associate will be responsible for data entry and assisting with data review related to the federal program compliances for the HEART Program. The Data Outcome Associate is also responsible for maintaining client files in accordance with agency policies, inputting client information in designated database(s) and gathering program metrics for weekly meetings. Oversee pre and post survey data collection processes for each virtual and in-person workshop. Ensure the quality of data being entered into data bases, intake forms, and questionnaires / surveys / waivers, etc. Confidentiality required. This position supports agency efforts to provide quality services to clients and report results in a manner that promotes effective decision making to achieve agency goals. Provide and assist with guidance on data management, evaluation component protocols and the development of the data collection process and procedures throughout the grant cycle.
Working Conditions

- Able to travel to multiple workshop locations around the Greater Houston Area.

ESSENTIAL FUNCTIONS
Job Function
Assist in recruiting of interested individuals to the HEART Program. Promote all agency and program services during recruitment events including weekend and evening events.Collects and verifies participant information at each workshop location.Complete data entry for new participants in the nFORM system, a federal management information system while maintaining federal PPI (Protected Personal Information) requirements.Creates physical client files as required by the grant including intake forms, waivers, completion certificates, etc. according to agency policies.Updates participant files and keep folders organized in a safe and secure filing cabinet.Attends workshops at the beginning and end of each cohort to assist with administering the pre / post assessments, intake forms, questionnaires, waivers, and any other recruitment forms as needed.Be knowledgeable of nFORM, input all data as required and monitors accurate numbers for performance measurements.Assists with quantitative and qualitative weekly, monthly and quarterly reports.Assists the Program Directors with the design and implementation of the evaluation / assessment strategic plan, training and technical assistance and any other instruments or testing tools deemed appropriate for program.Assists with conducting refresher sessions / trainings with team / staff on nFORM software updates as needed.Analyzes trends to identify for ways of improving data collection for the HEART Program.Responsible for testing all data equipment (tablets, computers, hotspots, etc..) and securing its functionality at various workshop locations prior to workshops.Report necessary modifications for the success of the data collection at workshops sites.Cooperates with staff members, site contact personnel, and evaluation team at each workshop site throughout the grant cycle.Evaluates program effectiveness and provide documentation regarding progress by maintaining accurate, complete, and authentic records as required by law, district policy and grant regulation / compliance.Cultivates high standards of communication with colleagues and program participants for the effectiveness of the program. The ability to work effectively with a wide range of constituencies in a diverse community.Establishes and maintains a healthy environment which is conducive towards the achievement of the program and agency’s mission.
