A company is looking for an LTSS Service Care Manager.

Key Responsibilities

Assist in developing, assessing, and coordinating holistic care management activities

Develop personalized service care plans for long-term care members

Educate members and families on available services and benefits

Required Qualifications

Bachelor's degree required with 2-4 years of related experience

RN or LCSW required for certain states

Safety-sensitive position in Arkansas requires a driver's license and background checks
