Key Responsibilities
Support care coordination teams in developing effective transition plans for members
Assist with member transitions based on enrollment or care service needs
Track and maintain transition metrics to identify trends and process improvements
Required Qualifications
Bachelor's degree required
2 - 4 years of related experience preferred
Equivalent experience may be considered in lieu of a degree
Experience in healthcare or care coordination is advantageous
Knowledge of community health services and transition planning is beneficial
