A company is looking for a Telephonic HSS Coordinator to engage members and coordinate care remotely in the Virginia market.

Key Responsibilities

Engaging members and families telephonically to coordinate services and treatment needs

Creating and revising care plans while advocating for members and collaborating with healthcare providers

Documenting assessments and ensuring timely case documentation in electronic files

Required Qualifications

Bachelor's Degree in Health or Human Services Field OR LMHP OR RN / LPN OR QMHP OR LMSW OR LBSW OR MSW OR BSW

2+ years of recent experience in case management, home care, or acute care settings

1+ years of experience working with individuals with complex medical or behavioral needs

Demonstrated solid computer proficiency (Microsoft Word, Outlook, and Internet)
