OVERVIEW : Responsible for developing plans for community-based health programs aimed at the prevention of disease and promotion of health and wellness. Recommends community health initiatives, recognizes policy implications, and identifies best practices after reviewing health literature and statistics. Seeks community health and wellness program related grant funding and develops grant proposals. Manages specific community health and wellness programs from planning and design through implementation and evaluation.
ESSENTIAL FUNCTIONS OF JOB :
Leadership and Management
- Prepares a comprehensive annual Community Health and Wellness Plan. Monitors and reports on performance related to the plan. Identifies tools, resources, and best practices to help improve the ongoing Community Health and Wellness Program.
- Works collaboratively with Community Health Team to create and implement strategic planning and effective program development.
- Interviews, selects, orients, trains, supervises, and evaluates Community Health and Wellness staff working in the areas of : WIC, Diabetes, Community Health and Wellness, Prevention Health, Garden, Complementary Care, and Transportation.
- Responsible for overseeing the work of Community Health Representatives and coordinating effective outreach activities.
- Develops and implements department policies and procedures. Ensures consistent application of all departmental policies.
- Reviews contracts, grants, and makes recommendations to the Health Services Director. Writes program descriptions and develops grant proposals.
- Negotiates and updates contracts as necessary.
- Research and analyze data required to assess Community Health and Wellness needs and the effectiveness of current programs. Prepare regular monthly performance and quality reports and other reports as assigned.
- Develops publications, grant applications, and reports through collaborative writing and editing.
- Identify opportunities for marketing health programs / services and implementing promotional activities.
- Identifies partnership and funding opportunities and promote relationship-building with other organizations, agencies, and stakeholders.
- Develop presentations and other materials for use during program-related events and meetings.
- Develops written and online tools for programmatic use including survey and evaluation instruments.
- Manages and builds relationships with key community stakeholders and contractors. Serves as programmatic liaison to external partners.
- Responds to community concerns in the delivery of Community Health services.
- Monitors and evaluates patient satisfaction. Tracks patient outcomes related to health services delivery. Makes recommendations and assists in the implementation of quality-of-care improvements related to Community Health and Wellness.
- Represents the Tribe to health services entities and leaders locally, regionally and nationally, as needed.

BA or BS degree from an accredited institution, in the field of business administration, public administration, health service administration, or allied field of study.Extensive, pertinent experience may substitute for education, as deemed applicable by employer.Must be oriented in customer service.Two (2) years progressively responsible management experience.Two (2) years’ experience working with health services programs.Two (2) years’ experience working with tribal organization(s).Grant and / or contract management experience.PHYSICAL DEMANDS :
Requires standing, sitting, stooping and bending intermittently.Requires ability to lift at least 10 lbs.Requires ability to hear and understand verbal instructions.Requires use of standard office equipment such as computers, telephones, fax machines, and photocopiers.CONDITIONS OF EMPLOYMENT :
Must have a valid Washington State driver’s license.Must pass a criminal background check.Must comply with federal laws and regulations as required by the Health Insurance Portability and Accountability Act (HIPAA).Must be able to travel to meetings, conferences and other events to represent the Tribe.As with all positions of the Lower Elwha Klallam Tribe this position is Indian Preference in hiring, in accordance with P.L. 93-638.
