Description :
The Remote Patient Monitor will help the patient experience including the ongoing treatment of patients.
Responsibilities :
- Maintains current knowledge about best medical practices.
- Maintains knowledge about the basics of chronic health conditions related to the devices and the operation of those devices : i.
e. glucose monitor, blood pressure cuff, scales.
- Work with a clinical team of professionals to consistently meet and exceed expectations for quality of patient care goals.
- Participate in all aspects of patient care and patient safety.
- Provide emotional support, validation, and listening to patients.
- Follow up with patients regularly via phone, video, and messaging apps as needed.
- Communicate inter-professional collaboration as needed for continuity of care.
- Work with the clinical team to review patient charts to confirm relevant and accurate documentation. Assist with regular auditing.
- Collaborate with the in-house clinical team to engage with patients who are being monitored between visits using our wireless monitoring devices.
- Contribute to process improvement, program design; continually looking for ways to build the best in patient care experiences for the community.
- Build and retain a thriving team culture.
- Contribute to quality improvement and process improvement initiatives.
Knowledge and Skills :
- Experience with Chronic Care Management in Athena EMR
- Experience in Remote Patient Monitoring Program.
- Ability to navigating computer platforms and familiar with smart phone functionality.
- Comfortable with telephone outreach and engagement
- Excellent listening skills
- Organized and efficient to maintain administrative and clinical duties.
- Excellent customer service skills, handles escalated situations with ease.
- Proficient with technology
- Tele-health experience.
- Strong technology experience
- Patient monitoring experience.
- Patient Education experience.
- Experienced with client interaction.
- Excellent communicator (written and verbal)
- Highly organized and efficient
- Detail-oriented and good at multi-tasking.
- Team Player
- Exhibits a strong sense of ambition, self-motivation and self-discipline.
Requirements :
- Medical Assistant, LPN, LVN, or RN
- EMR experience
Physical Requirements :
- Prolonged periods sitting at a desk and working on a computer.
- Prolonged periods of periods of standing and bending.
- Must be able to lift up to 15 pounds at times.
