Missouri Licensed Pediatric Care Manager

A company is looking for a Care Manager (RN) PEDS.

Key Responsibilities

Evaluate member needs and recommend care plans to address barriers to accessing appropriate care

Develop and collaborate on ongoing care / service plans with providers and community resources

Monitor member status and revise care plans as necessary to meet goals and needs

Required Qualifications

Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing

2 - 4 years of related experience in care management

Registered Nurse (RN) state licensure or compact state licensure required

Experience in pediatric physical and behavioral health preferred

Must reside in the state of Missouri
