We’re looking for a motivated, detail-oriented, and compassionate individual to join our Centralized Admissions Team, supporting hospital partnerships and helping patients access the care they need.

Position: Centralized Admissions Coordinator
Location: Primary location in Avon, CT. Must be willing to travel as needed.

What You’ll Do:

Review referrals from hospitals for all our skilled nursing facilities

Collaborate with hospital case managers and internal teams to ensure smooth, timely admissions

Make clinical and financial admission decisions that help patients transition to the right level of care

Plan, organize, and execute the admission process, ensuring compliance with all regulations and guidelines.

What We’re Looking For:

Strong communication and problem-solving skills

Strong attention to detail, follow-through skills, and ability to prioritize tasks.

Ability to assess clinical information and make sound admission decisions

Knowledgeable of admissions and discharge procedures, current laws, residents’ rights, and documentation requirements

Experience in healthcare, case management, admissions, or related field preferred
