RN - Case Manager

Our Client, a Healthcare company, is looking for a RN - Case Manager for their Fort Wayne, IN location.

Responsibilities :

• Collaborative work with the Care Team.
• Completion of comprehensive patient assessment including patient status to drive discharge.
• Driving stay progression
• Completion of discharge planning in a manner that expedites patients transition to the next level of care.
• Comprehensive and accurate Epic documentation
