We’re seeking a Board-Certified or Board-Eligible Physician in Internal Medicine, Family Medicine, or Geriatrics to deliver comprehensive primary care to aging adults across home-based and community settings in San Diego, CA .
What You’ll Do
- Serve as the primary care provider for assigned patients, managing preventive, acute, and chronic health needs.
- Perform comprehensive health assessments, order and interpret diagnostics, and guide personalized care plans.
- Collaborate with an interdisciplinary team of Nurse Practitioners, Physician Assistants, and Wellness Coordinators .
- Lead population health initiatives and quality improvement programs that enhance patient outcomes.
- Provide mentorship to mid-level providers and clinical support staff.
- Participate in regular provider meetings and professional development to advance clinical excellence.
Who You Are
MD or DO with an active California medical license .Board Certified in Family Medicine, Internal Medicine, or Geriatrics.Experienced in senior or geriatric care (preferred).Skilled in EMR documentation and team-based collaboration.Compassionate, communicative, and driven by patient-centered outcomes.Why Join Senior Doc Medical Group
