The Medical Records Specialist will be responsible for doc-linking non-electronic documents, processing end of episode documents, physician's orders, and medication profiles, along with other administrative duties as assigned.
Responsibilities :
- Primarily works in an office environment and answer phones according to agency policy
- Maintains the electronic medical record
- QA's daily notes for completeness and monitors the dashboard for incomplete documentation
- Assists with record audits
- Serves at the device expert in the office
- Send physician orders and track for signature, end of episode reports, medication profiles to patients, track Face-to-Face encounter documentation, doc-link all non-point of care documents, and other data elements as needed
- Makes admission packets as needed
- Maintains open lines of communication with all members of the healthcare team
- Communicate with staff and patients
- Runs hospital hold report and follows up with hospital to ensure services are prepared for patient resumption of care
- Ensures that transfer summary is sent to the physician and receiving facility within 2 business days for a planned discharge, or within 2 business days of becoming aware of an unplanned transfer
- Send discharge to the physician within 5 business days from discharge
- Assist other clerical staff members as necessary
- Follows agency policies and procedures
- Performs all other duties as assigned by the Administrator
- Able to lift 40 pounds from floor to shoulder
- Repetitive walking, standing, sitting, bending and use of hands
- Average hearing and vision
- Responsible for ensuring the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provides Age-Friendly Care Other duties as assigned
- Support direct care of patients by providing field staff scheduling and making adjustments to schedules to accommodate physician orders
- Review scheduling request report and makes changes to the schedule as requested
- Process therapy reassessment warning and ensures functional assessments are scheduled
