Advanced Practice Nurse - Family Practice

Job Summary :

To provide direct patient care in collaboration with the Archer Clinic physicians while integrating education, research, management, leadership, and consultation into the clinical role.

Essential Functions :

• Provides direct patient care, including ordering and evaluating of diagnostic tests and managing of procedures according to established clinical practice guidelines and in accordance with the privileges granted by the Archer Clinic.
• Identifies deviations from normal and consults, co-manages and refers patients as appropriate according to established clinical practice guidelines and in accordance with the privileges granted by the Archer Clinic.
• Serves as a resource for staff nurses and physicians in the Archer Clinic to facilitate the coordination of care and the movement of patients through the continuum.
• Provides individualized patient / family teaching, health counseling and health advocacy services to patients, their families and significant others.
• Completes all required documentation in a timely manner.
