Registered Nurse - Hospice Care Manager

As a Care Manager, you will be at the forefront of assessing, planning, coordinating, and delivering exceptional care within the community. • Conduct thorough assessments for patients with complex or high-risk needs.\n
• Create and implement personalized care plans aligned with each patient's medical needs and goals.\n
• Work closely with physicians, social workers, and other professionals to ensure comprehensive patient care.\n
• Facilitate seamless transitions of care across different settings and providers.\n
• Continuously evaluate and refine care plans to ensure cost-effectiveness and appropriateness of care.\n
• Address social determinants of health, including housing, transportation, and community resources.\n
• Encourage wellness and prevention strategies among patients.\n
• Maintain meticulous and accurate records of all clinical and financial activities.\n
• Act as a patient advocate while ensuring adherence to privacy and confidentiality standards.\n\nQualifications\n
• Associate's or Bachelor's Degree in Nursing (BSN preferred).\n
• Active and unrestricted Colorado RN License or Compact Multi-State License.\n
• Prior experience in Hospice Care or Home Health is preferred.\n
• HCHB (HomeCare HomeBase) experience is strongly preferred.\n
• Must have reliable transportation and valid auto insurance.\n
• Excellent critical-thinking, communication, and decision-making abilities.\n
