The Nurse Practitioner will facilitate HWRs efforts to relieve suffering, enhance comfort, promote quality of life and foster choice in the presence of an advanced serious illness for all patients and families. The practice is based on competent clinical knowledge and focuses on clinical practice, education, collaboration, consultation and research related to palliative care.
WHAT YOU WILL DO : Clinical Practice
- Participates in development and implementation of new programs that affect clinical practice
- Performs holistic patient assessment. Prescribes for symptom management and / or recommends symptom management to primary care providers.
- Performs medical history and physical exam in collaboration with the physician to develop a plan of care
- Coordinates care with team members and health care professionals involved in patient and family plan of care
- Reviews medical record including physician / consultant notes, lab and diagnostic test results. Formulates plan of care based on above data
- Provides follow-up to reassess the plan of care as determined by patients goals.
- Participates in analyzing and evaluating clinical practice standards in terms of cost and quality
- Provides independent advanced practice nursing care within the scope of practice as delineated in State of Ohio
- Participates in development and implementation of new standards of practice for hospice and palliative care
Consultation
Ensures quality care across the continuum as an expert in palliative careFacilitates and / or participates in Transdisciplinary team conferencesServes as a consultant to staff caring for patients in hospitals, long-term care settings and home settingsServes as an expert consultant to healthcare professionals at the local, national and international levels to influence and advanceclinical practice in palliative careEducation
Maintains a thorough knowledge of the current practice and literature relating to palliative care and facilitates the sharing of thatknowledge with staffEducates patients and families on applicable areas of palliative care, such as interventions for pain and symptom management, andchoice in end-of-life careEmpowers patients and families to identify and meet goals of careProvides education to HWR staff in areas of palliative careProvides education to outside agencies, hospitals and long-term care settings on principles of palliative careCollaboration
Maintains a standard care agreement with HWR Medical DirectorsWorks as part of a teamCollaborates with members of the palliative care teamCollaborates with health care professionals as appropriate within the communityFacilitates communication between nursing staff / physicians and patient / familyParticipates on HWR committeesParticipates in quality improvement activitiesMaintains and cultivates professional relationships in healthcare that strengthen HWRs strategic marketing planFulfills professional growth through involvement in professional organizations, presentations and publicationsResearch

Maintains a current Registered Nurse license in state of OhioMaintains a current Certificate of Authority in state of OhioMaster of Nursing DegreeMinimum of two years in clinical settingMust be flexible and have problem solving skills, prioritize and demonstrate healthy coping strategiesAbility to demonstrate excellence in written and oral communicationAbility to meet patient / family needs that require variable hoursKnowledge of hospice and palliative care clinical standards of practice, policies and proceduresAbility to practice in accordance with HWR clinical standards of practice, policies and proceduresAbility to establish and maintain collaborative working relationships with HWR employees, community and healthcare providersAbility to provide own transportationMust have and maintain a valid drivers license, good driving record and auto accident insurance at a level acceptable to HWR and / or HWRs insurer.DETAILS :
