Nurse Practitioner - GA Licensed

A company is looking for a Nurse Practitioner with experience in diabetes care.

Key Responsibilities

Provide video-based care to patients in the remote patient monitoring program

Deliver swift, empathic care and assess patients' competencies

Determine members' eligibility for the clinical program and handle clinical escalations

Qualifications

Education : Bachelor of Science in Nursing (BSN) & Master of Science in Nursing (MSN)

License : Active state license required; multi-state licenses preferred (GA, NC, NY, FL)

Minimum 3+ years of experience managing patients with diabetes

Spanish-speaking preferred; certification required
