General Summary of Position
Collaborates with providers to ensure safe and effective use of specialty medications and improves communication among the patient's healthcare team by reviewing and evaluating clinical guidelines formulating protocols conducting medication reconciliation creating patient care plans and providing patient consultation that will improve patient medication adherence health well-being and knowledge. Documents and reports on healthcare-related interventions and outcomes. Participates in the process of maintaining specialty pharmacy accreditation.
Primary Duties and Responsibilities
- Conducts medication order verification dosage regimen design effective administration and drug therapy monitoring services.
- Creates updates and maintains specialty medication patient care plans.
- Provides specialty medication education with patients and consultations with providers.
- Obtains patient and drug related information for initial assessment medication reconciliation patient care planning appropriate documentation and follow-up.
- Provides functional oversight for activities related to specialty pharmacy in the clinic setting.
- Documents all services according specialty pharmacy accreditation standards and payer specifications.
- Participates in collaborate practice agreements with prescribers to advance patient care
- Assists with the implementation follow through and administrative requirements associated with new clinical programs and all medications which require specialized monitoring.
- Coordinates with all appropriate clinical and administrative personnel on plans programs and operations to improve efficiency and productivity.
- Remains current with evidence based clinical practice guidelines so that patient care plans and consultations align with the latest research.
- Serves as a preceptor for student pharmacists summer interns and residents when applicable.
- Participates in the process of maintaining specialty pharmacy accreditation.
Minimal Qualifications
Education
Doctoral degree PharmD from an accredited college of Pharmacy requiredExperience
3+ years of direct patient care experience in a clinical role preferably in a specialty pharmacy required andAmbulatory care experience preferredLicenses and Certifications
PHARMD - Licensed Pharmacist in Maryland requiredEligibility for reciprocity to surrounding states as required for practiceCompletion of an ASHP accredited residency or other approved advanced training program Upon Hire preferred orBoard of Pharmacy Specialty certification (BPS) or other approved certification like Certified Specialty Pharmacist (CSP) AAHIVP preferredKnowledge Skills and Abilities
Verbal and written communication skills.Basic computer skills preferred.This position has a hiring range of : USD $120,702.00 - USD $238,222.00 / Yr.
