Care Transitions Nurse Manager - RN

HealthCare Support is actively seeking a Care Transitions Nurse Manager - RN to fill an opening with a prestigious Healthcare companyin The Bronx, NY

Daily Responsibilities for Care Transitions Nurse Manager - RN :

  • Will make field visits but will not carry their own caseload.
  • Assisting and overseeing some of the more complex cases - acting as a SME for Community Health Nurses in the field
  • Communicate with interdisciplinary teams, social workers, and dietitians, helping coordinate that care
  • Monday – Friday 8 : 30 am – 5 : 30 pm I On-Call Rotation
  • This is a field position

Required Qualifications for Care Transitions Nurse Manager - RN :

  • Active NY Registered Nursing license
  • 3+ years' experience as a Registered Nurse / RN
  • 2+ years of recent experience at a Certified home health agency (CHHA)
  • 1+ year of recent supervisory experience
