This position works within the Intensive Case Management (ICM) team. This team delivers community and office-based intensive case management services to individuals diagnosed with serious mental illness and needing support to progress toward their recovery goals. In addition to providing office-based services, the team is designed to provide outreach, engagement, and intensive case management services in community settings such as homes, shelters, employment sites, jails, and hospitals.
This position supports the ICM team in two locations - Annandale and Reston. Selected candidate will work half of the time in Annadale and the other half in Reston. Job duties will include checking vital signs; weighing clients; doing finger sticks to monitor blood sugar; administering IM injections; and unpacking and organizing medications. Interfaces with pharmacies and insurance companies as well as prescribers, to assist clients with medication needs. Uses excellent communication skills to interact with clients with mental health, substance abuse, and / or intellectual disabilities. Duties include case management. Additional duties may be assigned as needed for clinic operations.

Employment Standards
MINIMUM QUALIFICATIONS :

Graduation from an accredited college or university with an associate degree in nursing; plus two years of post-licensure experience as a nurse.

Upon completion of an underfill agreement (which will require that the employee meet the employment standards stated above in addition to obtaining competencies and licensures defined by the hiring agency) the employee will be elevated to the level of BHN Clinician / Case Manager.
NECESSARY SPECIAL REQUIREMENTS :
The appointee to this position will be required to complete a criminal background check, a Child Protective Services Registry check, driving record check, and sanction screening to the satisfaction of the employer. A TB screening upon hire and annually thereafter.

PHYSICAL REQUIREMENTS :
Ability to drive car, transport clients, make home / community-based visits, use personal computer and blackberry (BB), provide injectable medications, accurately perform tactile and auditory pulse, blood pressure and respiratory assessments. Communicate with client and staff in a timely and effective manner, observe and report on client appearance, mental status medical status and behavior. All duties performed with or without reasonable accommodations.
