What Youll Do

• Support individuals in their homes and community settings not a facility.
• Help clients participate in social recreational volunteer and faith-based activities.
• Provide reliable transportation and personal support as needed.
• Collaborate with families to design flexible meaningful daily and weekly routines.
• Document services progress and outcomes in Therap.
• Encourage independence dignity and self-advocacy.
