A company is looking for a Patient Enrollment Associate to connect families navigating dementia with the necessary resources and support.

Key Responsibilities

Conduct high-volume, empathetic outreach calls to families affected by dementia

Educate and enroll individuals in the dementia care program, ensuring understanding of benefits and eligibility

Collaborate with care teams and maintain accurate records of interactions and assessments

Qualifications

Bachelor's degree or equivalent (MSW preferred)

Minimum 1 year of experience in healthcare, social services, or patient outreach

Demonstrated empathy and comfort engaging with individuals experiencing cognitive decline

Comfortable using virtual tools and CRM / EHR platforms

Ability to work independently in a fast-paced environment
