who excels in this role : \nAccurately performs patient assessments and identifies patient needs\nIdentifies and initiates appropriate nursing interventions\nProvides care appropriate to condition and age of the patient\nPerforms timely and appropriate documentation relating to medical necessity in the medical record\nResponsible for completion and revision of the Interdisciplinary Care Plan for each patient\nPerforms timely and accurate QI assessments
