Job Summary :

  • Hybrid Setup - Combination of remote work and field visits. Visits could be up to 3 hours away from Charlotte. All travel, mileage, and lodging (if needed) is reimbursed. Great for people who love to travel!

Job Duties :

  • Managing a case load for healthcare members with long term care needs.
  • Monthly and quarterly member contact and will include 80% travel. 20% Remote role. Will require a driver’s license.
  • Member assessments and notes.
  • Complete assessments with members, caregivers, or providers to obtain information regarding client status, support system, and need for services for care plan development.
  • Monitor delivery of services and follow-up with members, caregivers, or provider s through in person visits and telephonic contact
  • Authorize and coordinate referrals for services.
  • Ensure provider services are delivered without gaps and identify functional deficiencies in plans of care.
  • Assist in coordinating the development of informal or voluntary services to integrate into the member care plan
  • Collaborate with discharge planners, physicians, and other parties to ensure appropriate discharge plan, care plan, and coordination of acute care and long-term care services!
  • Assist member with filing and resolving complaints and appeals.

    • Shift Times : Mon-Fri 8am-5pm with a 1-hour lunch break. OT may be required

    Pay Rate : $40 - $42 hourly (determined on experience)

    Required Skills :

  • Licenses / Certifications : Current NC State RN – no restrictions on license
  • 2 years of case management experience required
  • Must have Computer literacy, including with Microsoft Programs like Excel, Outlook, and Word
  • Must be willing to travel up to 3 hours from Charlotte

    • Preferred Experience :

  • Experience with a managed care organization (MCO)
  • Certified as a Case Manager (CCM) preferred.
  • Bilingual in Spanish and English, and able to translate fluently.
