Were seeking an experienced Infusion Pharmacist to join a growing infusion pharmacy team near Indianapolis, IN. This is a direct hire opportunity with a MondayFriday schedule, offering the chance to provide high-quality infusion pharmacy services in a collaborative and patient-focused environment.

About the Role :
The Infusion Pharmacist will provide safe and appropriate pharmacy services to patients in the alternate care setting, ensuring pliance with professional standards and regulatory requirements. This role involves coordinating patient services, reviewing therapies for appropriateness, dispensing medications and supplies, documenting patient progress, and providing education on proper medication administration.
Key Responsibilities :
Assess patient suitability for home infusion therapy and review prescribed treatments for appropriateness.
Develop patient-specific care plans and perform pharmacokinetic dosing as needed.
Enter and verify prescription orders accurately to ensure proper concentrations, stability, and storage.
Dispense medications and related supplies while ensuring all necessary authorizations are obtained.
Provide patient and caregiver education on medication use, infusion methods, and follow-up.
Maintain thorough documentation of therapy progress, laboratory results, and munications.
Oversee pharmacy support personnel and delegate non-clinical tasks appropriately.
Ensure pliance with USP
, state and federal regulations, and internal quality standards.
Participate in patient care conferences and on-call responsibilities as needed.
Utilize CareTend system for workflow and documentation.
Qualifications :
Bachelor of Science in Pharmacy or PharmD from an accredited school.
Current pharmacist license in the state of Indiana (or eligibility to obtain).
Minimum of 2 years recent experience in an acute-care setting OR 1 year of recent home infusion experience.
Strong knowledge of IV admixture services and infusion therapy.
Excellent munication, problem-solving, and organizational skills.
Ability to collaborate effectively with interdisciplinary healthcare teams.
