About the Role :
As a Direct Hire Infusion Pharmacist , you will provide safe, effective, and patient-centered pharmacy services . Responsibilities include assessing therapy appropriateness, dispensing medications, coordinating patient care, educating patients and caregivers, and ensuring pliance with state and federal pharmacy regulations . You will also support budget management, purchasing, and pharmacy operations as needed.
You will report directly to the Pharmacy Manager or Director and serve as a key contributor to patient care excellence .
Key Responsibilities :
Ensure pliance with pharmacy regulations, pany policies, and professional standards
Evaluate patient suitability for home infusion therapy and alternative care settings
Develop patient-specific pharmacy care plans and write medication orders
Perform pharmacokinetic dosing and remend appropriate medication dosages
Dispense medications, supplies, and troubleshoot IV administration devices
Document patient progress, municate with healthcare providers, and track therapy outes
Maintain sterile pounding activities in pliance with USP
Supervise and mentor pharmacy support staff, interns, and students
Participate in patient care conferences and on-call rotations
Serve as Pharmacist in Charge during temporary absences
Qualifications :
Bachelor of Science in Pharmacy required; PharmD or Masters preferred
Current Indiana Pharmacist license
Minimum 2 years acute-care experience or 1 year home infusion therapy experience
Strong knowledge of IV admixture services and infusion therapy
Excellent munication, multitasking, and problem-solving skills
Ability to supervise staff and collaborate with interdisciplinary healthcare teams
