Physical Therapist - Outpatient Clinic & Home Health - UP to 150K/YR

About Us : Connected Health Care is a leading recruitment and staffing firm specializing in healthcare placements. We are currently partnered with a prominent outpatient facility in San Bernardino County, CA, seeking a skilled and passionate Physical Therapist to join their dynamic team.
This position is up in the mountains.
Job Description : As an Outpatient Physical Therapist, you will be responsible for assessing and treating patients with physical impairments through the implementation of comprehensive therapy plans.
Your duties will include :
- Conducting thorough evaluations to determine patients' physical conditions and needs.
- Developing and implementing individualized treatment plans to promote mobility, reduce pain, and improve overall function.
- Utilizing therapeutic exercises, manual therapy techniques, and modalities to facilitate rehabilitation.
- Monitoring and documenting patients' progress, adjusting treatment plans as necessary.
- Collaborating with other healthcare professionals to ensure coordinated care and optimal patient outcomes.
- Providing patient education and home exercise programs to support continued progress and independence.
Requirements :
- Master's or Doctorate degree in Physical Therapy from an accredited program.
- Valid California state licensure as a Physical Therapist.
- CPR certification.
- Minimum of 1 year of experience in outpatient physical therapy preferred. New grads are possibly acceptable dependent on prior experience (PTA).
- Strong interpersonal and communication skills.
- Ability to work effectively both independently and as part of a multidisciplinary team.
