Travel LTAC RN – Grand Rapids, MI

Day Shift | 13 Weeks | 3x12s

HumanEdge Health is seeking a skilled and compassionate Travel Registered Nurse (RN) for a Long-Term Acute Care (LTAC) facility in Grand Rapids, Michigan . This 13-week day shift contract is an excellent opportunity for an RN who thrives in fast-paced, team-oriented settings and is passionate about delivering high-quality, patient-centered care.

As a Travel RN, you will be responsible for implementing and following individualized care plans, supervising ancillary staff, and ensuring a safe and therapeutic environment for all patients.

Key Responsibilities :

Complete comprehensive nursing assessments within 8 hours of patient admission

Develop and implement individualized care plans based on patient needs and interdisciplinary collaboration

Communicate effectively with patients, families, and healthcare team members regarding status and care goals

Provide timely documentation of assessments, interventions, and outcomes in the medical record

Educate patients and families to support health literacy and safe discharge

Supervise and delegate tasks to LPNs, CNAs, and new staff / orientees appropriately

Actively participate in weekly care conferences and daily rounds

Promote a safe, respectful, and efficient unit culture through leadership and communication

Maintain compliance with facility standards for infection control, appearance, punctuality, and professionalism

Identify opportunities for personal and professional development and participate in continuing education

May assume charge responsibilities as needed

Requirements :

Active RN license in Michigan (or compact license)

Minimum of 1 year of recent acute or LTAC nursing experience

BLS and ACLS certification (required)

Strong documentation, communication, and critical thinking skills

Ability to adapt quickly to new settings and teams

EMR proficiency preferred
