Position Description

This position is responsible for assisting patients and their families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care;

or may also be responsible for working with patients who are emotionally or mentally disturbed, collaborating with psychiatric and allied provider's teams in diagnosis and treatment plan.

They work closely with physicians and nursing staff to further their understanding of significant social and emotional factors underlying patient's health problem.

Work is performed under the general direction of the Director of Social Services, and requires frequent contact with representatives of other social service related agencies.

Major / Essential Functions

The employee in this classification will be an essential team member of the Texas Child Health Access Through Telemedicine (TCHATT) Program.

This is an innovative multidisciplinary treatment team designed to deliver behavioral health treatment and supports via telemedicine services to public school students in need of mental health services.

This Licensed Mental Health Clinician is responsible for the delivery of clinical services to children and their families as part of a team working together to provide rapid assessment, triage, stabilization, and connection to community-based providers.

This clinician works to assess the child's individual needs, coordinate services with schools and community providers and provides support during crisis.

This position will ensure the coordination between school, student, family, and community provider is collaborative, family centered, and committed to an agreed upon treatment plan.

  • Support the mental and physical health care of patients on an assigned patient caseload. Closely coordinate care with the patient's medical provider and, when appropriate, other mental health providers.
  • Provide patient education about common mental health and substance abuse disorders and the available treatment options.
  • Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications.
  • Provide or facilitate referrals to evidence-based psychosocial treatments (e.g. problem-solving treatment or behavioral activation) as clinically indicated.
  • Track patient follow up and clinical in conjunction with an EHR and stand-alone clinical management tracking system.
  • Document patient progress and treatment recommendations in EHR and other required systems so as to be shared with medical providers, psychiatric consultant, and other treating providers.
  • Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance abuse treatment).
  • Consult and collaborate with an interdisciplinary team of providers.
  • Administer and interpret assessments and measures.
  • Lead weekly evidence-based group psychological interventions with child / adolescent patients (and their caregivers) virtually.
  • Maintain communication (verbal and written) with caregivers, school personnel, primary care providers, etc.
  • Adhere to ethical and legal requirements.
  • Maintain communication with clinical supervisor.
  • Update monthly productivity reports.
  • Maintain and update professional knowledge and proficiency through continuing education, supervisions, staff meetings, workshops, and serving on special committees.
  • Follow policies and procedures of the department.

Required Qualifications

Education :

A minimum of a Master's degree in Social Work from an accredited Graduate School of Social Work.

License / Certification :

Licensed as a Social Worker by the Council on Social Work Education.
