Job Title : Nurse Practitioner or Physician Assistant (Part-Time)

Location : On-Site

Schedule : 2–3 Days / Week (Monday–Friday, occasional Saturdays)

Hours : 9 : 00 AM – 6 : 00 PM

Employment Type : Part-Time

About the Role

We are seeking a compassionate and skilled Nurse Practitioner (NP) or Physician Assistant (PA) to join our patient-centered team. You will play a vital role in delivering high-quality care through Annual Comprehensive Health Assessments and Well Child Visits , both in-person and via telehealth.

Your clinical expertise as a Nurse Practitioner or Physician Assistant will help identify active and chronic conditions, assess physical, mental, and social needs, and support patients with personalized treatment and counseling.

Key Responsibilities

• Conduct annual health assessments and well-child visits
• Obtain vital signs and calculate BMI
• Perform physical exams and review pharmacological therapies
• Counsel patients and families based on assessment findings
• Document visits using Electronic Health Records (EHR) – Practice Fusion preferred
• Deliver culturally sensitive care to a diverse patient population
