Conduct face-to-face and telephonic assessments with Medicaid Long Term Care / Comprehensive Program membersDevelop, implement, and monitor member care plansCoordinate services such as prior authorizations, PCP communication, skilled provider coordination, medication review, and community resourcesUtilize critical thinking, clinical judgment, and motivational interviewing to support member engagementPerform data review and documentation using care management toolsConduct multidisciplinary reviews to optimize member outcomesIdentify and escalate quality-of-care concernsProvide education, coaching, and guidance to empower members in healthcare decision-makingTravel to member homes, Assisted Living Facilities, and Skilled Nursing Facilities throughout Miami-Dade CountyCase Management Coordinator Requirements
Bachelor’s degree in Social Work or related field requiredCase Management experience requiredLong Term Care experience preferredProficiency in Microsoft Office (Excel, Teams, Outlook)Strong written and verbal communication skillsAbility to multitask and adapt in a fast-paced environmentAbility to work independently and maintain organizationCandidate must be fully bilingual (English & Spanish) — able to speak and write fluentlyValid High School Diploma or GED (verifiable)This is NOT a nursing role — no RNs or LPNsPreferred Qualifications
