Job description

A-Line Staffing is now hiring a Healthcare Consultant I (Case Management Coordinator) for a major Fortune 6 healthcare organization. This position offers long-term career growth and the opportunity to support members throughout Miami-Dade County.

Case Management Coordinator Compensation

  • Competitive pay rate $34.13 / hr

Case Management Coordinator Highlights

  • Fully remote role with 50–75% travel for face-to-face member visits
  • Full-time, 40 hours per week
  • Monday–Friday, 8 : 00AM–5 : 00PM EST
  • Training conducted remotely via Microsoft Teams
  • Candidate must reside in Miami-Dade County
  • Bilingual (English & Spanish) fluency required
  • Case Management Coordinator Responsibilities

  • Conduct face-to-face and telephonic assessments with Medicaid Long Term Care / Comprehensive Program members
  • Develop, implement, and monitor member care plans
  • Coordinate services such as prior authorizations, PCP communication, skilled provider coordination, medication review, and community resources
  • Utilize critical thinking, clinical judgment, and motivational interviewing to support member engagement
  • Perform data review and documentation using care management tools
  • Conduct multidisciplinary reviews to optimize member outcomes
  • Identify and escalate quality-of-care concerns
  • Provide education, coaching, and guidance to empower members in healthcare decision-making
  • Travel to member homes, Assisted Living Facilities, and Skilled Nursing Facilities throughout Miami-Dade County
  • Case Management Coordinator Requirements

  • Bachelor’s degree in Social Work or related field required
  • Case Management experience required
  • Long Term Care experience preferred
  • Proficiency in Microsoft Office (Excel, Teams, Outlook)
  • Strong written and verbal communication skills
  • Ability to multitask and adapt in a fast-paced environment
  • Ability to work independently and maintain organization
  • Candidate must be fully bilingual (English & Spanish) — able to speak and write fluently
  • Valid High School Diploma or GED (verifiable)
  • This is NOT a nursing role — no RNs or LPNs
  • Preferred Qualifications

  • Experience in Medicaid, LTC, or community-based case management
  • Experience conducting home visits or field-based assessments
  • Knowledge of community resources in Miami-Dade County
