JOB DESCRIPTION
Description
- Provides patient care, under the supervision of a Case Manager : Nurse or therapist.
- Provides personal care / companionship as directed by plan of care
- Provides light housekeeping / light meal prep tasks as directed by plan of care
- Effectively communicates both verbally and in writing
- Initiates and maintains positive relationships with patients / customers and co-workers.
- Observes and reports patient status changes including skin and psychosocial.
- Takes responsibility for self-development and supports a learning environment.
- Displays commitment to the mission of UHHS / UHHCS and its values.
- Performs other duties as assigned.
- Complies with all policies and standards.
- For specific duties and responsibilities, refer to documentation provided by the department during orientation.
- Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients.
- Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
- This position will be 100% traveling to the patients homes to provide care.
- This will be the day / evening rotation.
- This position will be in Home Care Services.
- Full time 40 hrs. per week.

QUALIFICATIONS
Qualifications
- High School Equivalent / GED Required.
- Completes required continuing education requirements to maintain certification.
- 1+ years Home care experience.
- State Tested Nursing Assistant (STNA) or equivalent certification.
- Basic Life Support (BLS).
- OH Driver’s License (Valid) with car insurance coverage required.
- Successful completion of agency written test and competency evaluation of skills in the field Required.
