Home Infusion Clinical Liaison

Job Description

As a Home Infusion Clinical Liaison, you will play a crucial role in creating and maintaining positive referral relationships to increase home infusion referrals within assigned accounts.

You will work with discharge planners, facility staff, physicians, and other individuals to assess the appropriateness of home infusion, facilitate patient care transitions onto service, and ensure proper documentation for ordered infusion therapy.

Your consultative functions will include providing consultation and informational presentations regarding home infusion therapy, serving as a clinical resource and educator for ordering providers, facility staff, patients, and families on home infusion therapy.

Required Skills and Qualifications

• Education sufficient for licensure
• Oregon Registered Nurse License upon hire
• National Provider BLS - American Heart Association upon hire
• 1 year full-time experience in a clinical role
