Manager, Clinical Nursing

Commonwealth Care Alliance's (CCA) Care Delivery program is responsible for providing care delivery and care management to a specific panel of high risk and complex people. This panel of patients incorporates the care of individuals with significant medical, behavioral, and social complexities that require intensive care management and care delivery. Care Delivery comprises of multiple program lines including CCA Primary Care. Founded in 1977, the practice known today as CCA Primary Care is a Massachusetts-based provider recognized for its work in the care of individuals facing complex challenges. Within Care Delivery, the Manager, Clinical Nursing is responsible for the oversight of clinical care delivery and complex care management within the interdisciplinary team. The Manager, Clinical Nursing is specifically accountable for assigned interdisciplinary team members and their performance by ensuring specific key performance metrics and daily clinical expectations are met. These expectations include oversight of Care Delivery clinical operations for care delivery and care management, patient experience, and interdisciplinary team collaboration.

Reporting to a member of Care Delivery Leadership, the Nursing Manager is the nursing leader of the practice. Responsible for all nursing and nursing support functions within the practice, the Nursing Manager will collaborate closely with other members of the management team. This role is full-time on site in one of our clinics and will assume the primary responsibility for clinical operations. Working Conditions :

This is a full-time clinic-based position. It is patient-facing. After on-boarding, there may be 1 day / week the role can work from home if adequate on-site coverage can be ensured. Laptop, Monitor, Headset, Phone, Microsoft Office, ShoreTel, Skype for Business, Guiding Care, EClinicalWorks, all programs that are related to documentation. Standard office conditions. Work locations include residential and community sites. This position requires in-person visits to patients in their homes and will support patients across various locations. Valid driver's license with no restrictions. Ability to be active and mobile across assigned catchment area. COVID-19 vaccination is required. Compliance with all Community Clinician Occupational Health Requirements

Required Education (must have) : Bachelor's degree in nursing required

Desired (nice to have) : Master's degree in nursing Preferred

Required Licensing (must have) : Current and active state RN License with licensure in good standing in the state served Current CPR or Basic Life Support (BLS) Certification Required

Required Experience (must have) : Meaningful clinical experience including : 3+ years' experience as a Registered Nurse in a high-touch clinical environment or home care, OR 2+ years caring for patients / members with complex medical, behavioral health, and social needs Primary Care experience

Required Knowledge, Skills & Abilities (must have) : Must have the ability to handle stressful emergency situations calmly and effectively with patients, families, and health professionals. Must have the ability to de-escalate situations. Must have the ability to effectively work with patients with various cognitive, physical, and behavioral abilities Capacity to function independently within licensed scope of practice. Critical clinical thinking skills required. Excellent communication skills, i.e., listening, oral and written English skills while conversing. Willing to learn and utilize telehealth technologies (video, chat, etc.), when appropriate, for a variety of clinical care and care management services.
